Quality Reporting of Registered Products

Report a quality, labelling, packaging or efficacy problem with a product registered by the Authority.

Use this channel to report a quality problem with a registered product — a defect in the product itself or its packaging, labelling or apparent efficacy. Reports from healthcare professionals are a primary source of quality signals for the Authority.

Before you report

Confirm the product is registered with the NPRA. A registered product carries a MAL number and appears in the QUEST3+ product search with an active status.

What to prepare

  • Product details — full product name, brand, registration (MAL) number, batch number, manufacturing and expiry dates, and the manufacturer or distributor.
  • The defect — what is wrong, in as much detail as you can give: discolouration, foreign particles, faulty packaging, unusual smell or taste, unexpected appearance.
  • Evidence — photographs of the product, its packaging, the defect and the batch and expiry details. Keep the product and its packaging in case they are needed for inspection.
  • Your contact details — name, telephone number and email address.

Unregistered products

If the product does not appear in QUEST3+, or appears to be counterfeit or illegal, it is outside this channel. Report it to the Pharmacy Enforcement Division through the public complaints system, giving as much detail as you can about the product and where it was obtained.
